Activity 33.4: Critical thinking

There are no right or wrong answers for this activity. Students are expected to review their current discharge procedures and think how they can best ensure continuity of care and safe discharge.

  • What nursing assessment models and tools are used? Do they include assessments of the skin other than for wound care or pressure care?
  • Consider the routine or rituals of the clinical area. Do they allow individual preferences for hygiene purposes?
  • What toiletries does the clinical area use if patients don’t have their own? Have a visual check on the different cleaning agents that the patient can potentially be exposed to in the clinical area.
  • Understand the nursing hierarchy for reporting concerns. Also consider the support from mentors, practice facilitators and educational supervisors.
