There were many types of abuse uncovered during the inquiry. These mostly centred on institutional abuses where essentially the trust was providing a service for the trust rather being patient centred. This fostered a culture to develop within the trust where poor practice and abuse was allowed to flourish unchecked. Widespread neglect of patients’ needs was evident as well as examples of physical and psychological abuse.
