Case study 9.1: Jane

Communicating effectively with Jane to develop a caring and therapeutic relationship will help Alex assess and understand the unique challenges and needs of Jane.  He should uphold his professional values by making the care of Jane a main concern in the planning and implementation of her transition

Alex could uphold his professional values by creating an open and honest approach to managing the transition of Jane from children’s to adult services. He can help uphold these values  collaboratively across services with Jane to co-produce a plan of care to meet the specific care needs of Jane

Case study 9.2: Rafal

Rafal could use the research to communicate the risks and impact associated with sleep deprivation of patients in hospital. He could also use the evidence base to propose a solution to minimising the environmental noise on the ward. Supporting the need for change and the aims and objectives for the change from a robust evidence base will improve Rafal’s chances of influencing approval for the change.

Following the approval of the plan, Rafal should use the planning stage plan to identify who will need to be involved with the change, what change would be   possible, how it can be achieved and how the quality outcomes will be measured. He will need to consider what tools that he can use in this stage to help identify and mange any risks associated with the change. 

In the evaluation stage, Rafal will need to use appropriate tools to determine if the intended quality outcomes of improving patients sleep through the reduction of environmental noise were achieved.
