Membership and copyright forms can be downloaded from the Internet at the following Web sites.
ASCAP
At the American Society of Composers, Authors and Publishers (ASCAP) Web site, you can download the ASCAP Writer Application, the ASCAP Publisher Application, and the membership agreement form, which appears at the end of both the Writer Application form and the Publisher application form. At the ASCAP home page (www.ascap.com), click on the Join ASCAP link, or go directly to that page using this URL: www.ascap.com/about/howjoin.asp
BMI
Go to the Broadcast Music Inc. (BMI) Web site (www.bmi.com) and click on the “join” link to determine if you are eligible to join BMI as a publisher or writer/composer. To join online, you must meet several requirements: (a) Technical requirements have to do with your computer operating system, ability to read pdf files, and ability to print the application. (b) Professional requirements include having your work commercially recorded and released or having written/co-written or composed a song that is likely to be performed on radio, TV, the Internet, and the like. (c) Legal requirements have to do with age and affiliation or past affiliation with a performing rights organization.
Copyright Forms
Paper copies of the U.S. Copyright Forms PA (for a work of the performing arts) and SR (for a sound recording) are being phased out. To file an electronic application through the Copyright Office online system, go to www.copyright.gov/eco. There you will find links to eCO Tips, eCO FAQ, and the eCO Tutorial. Alternatively, to register basic claims use the new fill-in Form CO, which replaces Forms PA and SR. The form, along with Tips, FAQs, and a Tutorial, can be found at www.copyright.gov/forms
